Admission

The admission counter is located at Lobby B, Ground Floor, Block B. (Open 24 hours daily)

All admissions require a deposit and/or letter of guarantee

The patient is required to fill up the consent form after the doctor has explained the procedure to be performed

Bring:

• Original identity card/ passport
• For children below 12 years old: birth certificate/ mother’s identity card

Birth Registration

All births in Malaysia have to be registered at one of the Jabatan Pendaftaran Negara. Upon registration, the newborn will be issued a Birth Certificate.

Bring:

• Original and photocopy Identity Cards/entry permit/passport of both parents
• Original and photocopy Marriage Certificate of parents
• Duly completed Form JPN.LM01
• Notification of Live Birth issued by the hospital
• Prenatal Card/Book/Buku Merah for pregnancy checkup
