Itchy anus

I have anal itchiness for months and worsening. Not responding to oral antifungal, steroid creams. Itch is not relieved by oral antihistamineI do practise proper hygiene, tried loose clothing, wipe it dry after bowel movement. What is the cause of my intense anal itchiness?
