Medical Reports

Medical reports are issued upon request by the patient or authorized representative, subject to hospital policy and doctor’s approval.

A completed application form, supporting documents, and payment are required. Estimated processing time is within four (4) weeks, subject to variation based on case requirements and reports will be released only to the applicant or authorized party.

To apply for a medical report, the applicant must complete the Application & Consent To Release Medical Information Form and submit it together with the required supporting documents where applicable.

  1. If you are the patient aged 18 years or older:
    1. Application & Consent To Release Medical Information Form
    2. Copy of NRIC (Identity Card) showing both the front and back, or a copy of the passport biodata page
    3. Purpose-Specific Forms (if applicable)
      • Insurance claim forms, KWSP / PERKESO / SOCSO forms, lawyer letters, embassy letters etc., depending on the report use
  2. If the patient is under 18 years old or mentally incapacitated:
    (A parent or legal guardian must apply on behalf of the patient)
    1. Application & Consent To Release Medical Information Form
    2. Copy of Patient’s Birth Certificate
    3. Copy of NRIC (Identity Card) showing both the front and back, or a copy of the passport biodata page
    4. Copy of Parent / Guardian’s NRIC (Identify Card) showing both the front and back, or a copy of the passport biodata page
    5. Purpose-Specific Forms (if applicable)
      • Insurance claim forms, KWSP / PERKESO / SOCSO forms, lawyer letters, embassy letters etc., depending on the report use
  3. Representative (Agent / Lawyer / Insurance / Employer):
    (A representative may apply on behalf of the patient when authorized to do so)
    1. Application & Consent To Release Medical Information Form
    2. Letter of Authorization (LOA) – signed by the patient
    3. Copy of patient’s NRIC (Identity Card) showing both the front and back, or a copy of the passport biodata page
    4. Copy of Representative’s NRIC (Identity Card) showing both the front and back, or a copy of the passport biodata page
    5. Purpose-Specific Forms (if applicable)

  4. For Deceased Patient(s):
    (Medical report is requested after death)
    1. If the requestor relationship to the patient is spouse/parent:
      1. Application & Consent To Release Medical Information Form (signed by spouse/parent)
      2. Copy of Death Certificate
      3. Copy of Marriage Certificate (to verify relationship between the deceased and spouse)
      4. Copy of Birth Certificate (to verify relationship between the deceased and parent)
      5. Copy of parent/spouse’s NRIC (Identity Card) showing both the front and back, or a copy of the passport biodata page
      6. Copy of deceased’s NRIC (Identity Card) showing both the front and back, or a copy of the passport biodata page
    2. If the requestor relationship to the patient is sibling/child:
      1. Application & Consent To Release Medical Information Form
      2. Letter of Authorisation (LOA) or Grant of Probate (for non-Muslim) or Surat Kuasa Mentadbir (for Muslim) – official court document stating the appointed administrator of the deceased’s estate
      3. Original Consent Letter signed by the appointed administrator
      4. Copy of Death Certificate
      5. Copy of sibling/child’s NRIC (Identity Card) showing both the front and back, or a copy of the passport biodata page
      6. Copy of deceased’s NRIC (Identity Card) showing both the front and back, or a copy of the passport biodata page

Standard Medical Reports and Insurance Claims are processed within an estimated timeframe of four (4) weeks from the date all required documents are submitted.

While we strive to meet the standard completion timeframe, certain circumstances may result in delays, including:
  • Doctor unavailability (e.g. on leave or attending conferences)
  • Patient currently admitted or hospitalised
  • Upcoming clinic appointments
  • Requests involving multiple reports from different doctors
  • Doctors with limited clinic sessions at Pantai Hospital Kuala Lumpur
  • Visiting or locum doctors without fixed clinic schedules at Pantai Hospital Kuala Lumpur
  • Submission of incomplete supporting documents

Specialist medical reports or expert opinion cases may require a longer processing time, as a review at the Specialist Outpatient Clinic may be necessary. This may occur following patient discharge, upon referral by a legal firm, or when an open-dated clinic review has been scheduled.

Applicants may receive the completed medical report through one of the following methods:
  1. Collection at Medical Report Room, Level 5, Block A
    • Original NRIC or Passport is required for identity verification
  2. Email Delivery
    • The report will be sent to the email address provided in the Release Form
  3. Courier / Mail Delivery
    • The report will be couriered to the address stated in the Release Form

Yes, If collecting on behalf of the patient/requestor, an authorization letter signed by the patient is required.

No. The hospital will only release the medical report to the applicant or the authorized requestor. The applicant is responsible for submitting the report to the respective insurance company, SOCSO, EPF, or any other third party.
